Primary Care Network (PCN) teams must update the Primary Care School of any changes to their PCN, that may impact the learning environment.
Changes affecting practice(s) and the PCN
Submit the PCN learning environment significant change reporting MS Form or scan the QR code.
Responses will be reviewed on a case-by-case basis, and your team will be contacted if follow up action, or clarity, is required.
Staffing changes within the PCN
Please email england.primarycareschooltvw.se@nhs.net if the following colleagues change within your PCN, so we can ensure our contact lists remain up to date:
- PCN administration lead
- PCN clinical director
- PCN education lead
- Practice manager(s)
Educators / Supervisors for wider workforce students / learners
The Primary Care School will check appropriate supervision is in place at the point of arranging a student / learner placement, we do not hold a database of educators / supervisors.
Please notify us via england.primarycareschooltvw.se@nhs.net if there is an issue affecting availability of an agreed educator / supervisor during placement.
GP Clinical or Educational Supervisors
Please notify your local patch team, or GP School team as appropriate.
