The additional hub site approval process is for Primary Care Network (PCN) learning environments that were not able to request approval for all GP practices, within their PCN footprint, at the point of initial approval. This might be for a variety of reasons including:
- Changes to the PCN footprint meaning a practice has joined or left the PCN (please notify us of changes)
- Changes to the PCNs capacity
- Previous non engagement
Guidance for completing the additional hub site approval form
To request approval for additional hub sites as part of your approved PCN-wide learning environment:
- Provide organisational details for new hub sites only (page 2-4)
- Fully complete the quality self-assessment (page 5 onwards)
This form is for use by approved PCN learning environments only.
The self-assessment is split into two sections – content from your initial learning environment approval document, and evidence to support the addition of your new hub site to the learning environment.
Please reference all wider workforce students / learners, and medical students, foundation doctors, GP trainees and GP retainers, that have been hosted across your PCN in the last 12 months to provide an overview of the learning environment, and potential for interprofessional educational opportunities.

Applications for additional hub site approval will be reviewed on a case-by-case basis by the Learning Environment Lead for the PCN, and one of our approval / verification panel chairs.
You will be notified of the outcome of the application.

Definitions
- Hub site: within this document, the term hub site refers to a GP practice that constitutes a student / learners main placement base
- Spoke site: within the scope of the student / learners allocated long placement, they may have a planned shorter ‘spoke’ or ‘alternative’ placement experience. Think of this as a wheel of a bicycle – hub at the centre and the spokes coming from the hub to services available within the PCN. Examples of spoke placements might include a visit to a care home or community pharmacy, or a day spent with local community teams
